It is a misconception among the public as well as the profession especially youngsters that Homoeopathy is more effective in chronic disorders rather than in acute disorders. From time immemorial Homoeopathy has proved its efficacy in all sorts of acute disorders including those diseases pronounced to be incurable by the other systems of medicine.

Though most of the physicians are interested in the treatment of chronic disorders only, certainly there are some occasions where one has to meet emergencies like epidemics, injuries or even acute exaggerations of chronic disorders. One may simply send the case to other systems of medicine but if we are confident of treating the acute episode, surely it will be reward by way of getting more patients to our treatment along with monetary reward.

To treat emergencies is very easy when compared to chronic ones since the symptomatology is very clear and the response to the drug can be assessed within a few hours. If one can fix the ‘Genus Epidemicus’ it becomes easier to treat the acute diseases which also help to prevent the disease and gain confidence of the patients. To give few examples, Verat alb has cured hundreds of cases of diarrhoea and vomiting for more than ten years. Hyos has cured more than twenty five direct cases of ‘Japan B. Viral Encephalitis’ in epidemics including a case of ‘Post-Encephalitis Sequelae’ with the same drug while the government supplied Bell, Calc carb and Tuberc to prevent the disease throughout the state. Only 4 cases are cured during the first outbreak of Japan B. Viral Encephalitis’ in 1979. The cases recovered rapidly without any sequelae left behind. For many years I found Bell is ‘Genus Epidemicus’ for fevers of respiratory origin and Ars alb for ‘Enteric

fevers in our region.

Treating acute emergencies will demand thorough knowledge of all subjects, close observation of the patient and the art of administering the medicine without any delay by way of consulting the Repertory or Materia Medica. One can find a few drugs which are necessary for day today practice by treating the same with constant observation and application of our system.

Lycopodium clavatum in a Case of Gilbert’s syndrome

Gilbert syndrome is a presumably lifelong disorder; it is a common, mild liver disorder, in which the liver doesn't properly process bilirubin.

Gilbert’s syndrome affects 3 to 10 percent of the U.S. population. More males are affected then females. Because males tend to produce higher levels of bilirubin, a male-to-female ratio is ranging from 2-7: 1, and occurs in all races (2). This Syndrome may become apparent shortly after birth; it may not be recognized for many years. It is rarely diagnosed before puberty because production of bilirubin increases at this point. It is usually diagnosed around puberty, possibly because of the inhibition of bilirubin glucuronidation by endogenous steroid hormones. In older persons, the diagnosis is usually made when unconjugated hyperbilirubinemia (1, 3).

In some early reports of Gilbert Syndrome autosomal dominant disease had been suggested (7), Genetic basis was achieved only in year 1995 (2), but a clear genetic pattern is often hard to establish. Sometimes it is misdiagnosed as chronic hepatitis. Dehydration, fasting, viral infection, Menstrual periods and Stress such as trauma and overexertion are Other Common causes (2).

30% of patients are asymptomatic, Fatigue, nausea, abdominal pain and diarrhea may also occur but these are much less definitive in determining the diagnosis. The mild jaundice may be especially evident. Pigmented skin thickenings, similar to moles and soft yellow spots (xanthelasma) on eyelids may occur. An increase in pigmentation on exposure to light and heat are other symptoms of this liver disorder. Enlargement of the liver and spleen are rarely seen.

Gilbert syndrome can be diagnosed by a thorough history and physical examination and confirmed by standard blood tests. Repeated investigations and invasive procedures are not usually justified for establishing a diagnosis (3).

Homeopathic concept: Gilbert syndromes fundamental cause can be psoro syphilitic with few exiting causes like stress, strain, and exposure to heat. In a day to day practice we are missing this syndrome and treating for other hepatic causes. Few months back a case was referred to me with clinical diagnosis as Gilbert syndrome. The following is the case.

Complaints developed gradually. Two years back the patient developed nausea, took allopathic medicines with a family physician for 8 months with partial relief, then he consulted gastroenterologist, he after doing investigations he diagnosed the condition as Gilbert’s syndrome. In spite of his treatment for over a year his symptoms persisted with every three months remission. With this patron of symptoms he developed anxiety about his health; Contradiction < with out work stress, he opted for Homoeopathic treatment. Nausea on and off since 2 years , no particular time , < fatty food ( Puri , Biriyani etc ) on and off since 2 years , desire fat food 8months , food > warm food , < riding in a car or bus since 6 months . He developed nausea, < fat foods. Of late he developed anxiety about his health; Contradiction - intolerant due to present illness.

Case was reviewed on 05/04/07 (6months) patient was able to tolerate all food, earlier every 3months remission was there, which was not seen.

Allergies are a common and significant cause of morbidity, lost-productivity and increased health care. It is not uncommon to see children on antibiotics, antihistamine cough syrups, tonic and off-and-on bronchodilators for years together without any referral to a specialist. This irrational treatment not only results in disease and economic burden on the family but also lost opportunities of growth and development for the child. A respiratory allergy is a specific immunologic response to a normally harmless allergen. Some people with allergy develop asthma and some allergic rhinitis.

Homoeopathy offers very effective treatment for all allergic diseases such as various skin diseases like urticaria, eczema, and contact dermatitis, as well as given unbelievably miraculous results in asthmatic bronchitis etc. Allergic rhinitis is defined as an allergen-induced inflammation of the membranes lining the nose and the sinus, often due to an allergic reaction to pollen, dust or other airborne substances. The various types of allergic rhinitis include Atopic Rhinitis, Seasonal Allergic Rhinitis (also known as hay fever) and Perennial Rhinitis (year round).

Of these, allergic rhinitis remains the common cause of morbidity and social embarrassment. Recent surveys revealed a four-fold increase in the number of consultations with general practitioners, although the reasons are unclear.

RAPID increase

A rapid increase in allergic diseases like asthma, allergic rhinitis, atopic dermatitis and food allergies has been observed worldwide. Globally, it has been noted that allergic rhinitis affects 24% of the population n the United Kingdom, 20.6% in Norway, and 19.6% in Germany and 26% of the population in India. Today, the incidence of asthma-related cases is 10-12 percent. In terms of the age-groups affected, about 70 per cent of patients develop allergic rhinitis before they are 30 years of age and, the adolescent children have greater incidence with allergic rhinitis occurring between 8-11 years of age. Over $600 million is spent annually in the management of this disease by United States which does not include the costs of the 2 million lost workdays, 3 million lost school days and 28 million days of decreased productivity from the symptoms of the disease and/or side-effects of the medications used to treat them.

Heredity plays a role in the development of allergies. The risk of the developing allergies is 18% if neither parent is a topic, 20% if one parent is atopic and 50%, if both parents are atopic.

Common Causes

Irritants like smoke, perfumes, strong smells, air pollutants and temperature changes, dust mites (see figure), polluted areas having high levels of sulphur dioxide and nitrogen diospets, moulds growing on wallpaper, upholstery, carping and plants within the house are found to cause allergic rhinitis. Grasses and trees that release minute pollen grains are also known to cause allergies. Allergies contracted from grass are experienced more in the late afternoon while that from mould spores may peak on dry windy afternoons or on damp or rainy days in the early morning. Intake of alcoholic drinks like wines and beer, sea foods, chlorine in swimming pool water, drugs like birth control pills and female hormones, some drugs used for blood pressure control, aspirin and chronic use of decongestants in the nose can also produce rhinitis.

Perennial rhinitis: common causes are dust mite found in many homes, and domestic pets. Salivary protein of cat, allergens from cockroaches etc. also cause perennial rhinitis. Occupational rhinitis is caused by an agent inhaled at the work place.

When an allergen such as pollen or dust is inhaled by a person with a sensitized immune system, it triggers antibody production. These antibodies mostly bind to mast cells, which contain histamine and when the mast cells are stimulated by pollen and dust, histamine and other chemicals are released. This results in itching, swelling, and mucus production. Symptoms vary in severity from person to person with very sensitive individuals experiencing hives or other rashes.

Symptoms of allergic rhinitis include frequent or repetitive sneezing (it is an allergic reaction to something in the air); a runny or congested nose; itching in the nose, eyes, throat, or roof of the mouth; plugged-up ear canals; sore throat; occasional nose-bleeds; impaired smell and wheezing. Sometimes, cross-reactive allergy to some fruits such as the skin of apples or potatoes occurs because of similarities in the proteins of the pollen and the food. Additional symptoms include fever, fatigue, flushing and irritability. Many patients develop watering, itching and redness of the eyes, collectively known as allergic rhino conjunctivitis.

Almost 20-50 percent of patients with allergic rhinitis go on to develop asthma as allergic rhinitis is a risk factor for developing asthma. A large number of children who start with asthma also develop allergic rhinitis.

Diagnosis

The most important diagnostic tool is the history of the illness. The timing of symptoms also helps in making the diagnosis. Allergic rhinitis that appears seasonally is almost always due to pollen; and outdoor allergens can be detected by conducting a skin test when the patient is not taking antihistamines. Skin-testing should also be avoided in patients with extensive eczema. For such patients, ELISA and RAST are two commonly used methods that measure the presence of food-specific IgE I the blood of the patients. However, they are more costly than skin tests and also take more time and a simple method for detecting common allergens.

Complications of allergic rhinitis lead to ear infections due to blockage of the Eustachian tube, recurring headaches, reduced concentration, reduced hearing, appetite, and growth.

Symptoms of Asthma

Asthma is a chronic disease in which the airways of the lungs become inflamed or narrowed, resulting in disruptions to normal breathing patterns, often called attacks’ or “episodes’. The level of severity of asthma suffered by each individual, and further, the severity of each attack, varies greatly. Genetic predisposition toward developing asthma and environmental factors play a role in the actual development of the disease. Symptoms of asthma include wheezing, shortness of breath, a feeling of tightness in the chest, and coughing, which are due to a narrowing of the bronchial passages in the lungs, and to excess mucus production and inflammation. Asthma can be disabling and sometimes fatal.

Treatment: A detailed clinical history of the patient’s illness will identify the likely cause of rhinitis.

Medical philosophy is increasingly coming to the conclusion that the mere treatment of symptoms and organs can only help temporarily and that it is the healing power of the body as a whole that has to be enhanced. Homoeopathy believes in a holistic, totalistic and individualistic approach which provides relief, free of side-effects.

Clinical trials conducted on allergic disorders like atopic dermatitis and childhood asthma in Hyderabad showed encouraging results with homoeopathic remedies. The same was published in an international journal Homoeo Links.

A few homoeopathic remedies with their indications

Arsenicum album: sneezing (with every change in the weather), watery discharge from the nostrils, excoriation of the upper lip, itching, burning and watering from eyes.

Allium Cepa: Prepared from red onion has this excoriating nasal discharge, there is more lachrynation which is bland. The discharge ceases when the patiehnt goes into the open air, but returns when entering a warm room again.

Euphrasia: is similar to Allium cepa, but it has profuse lachrymation, while having bland nasal discharge.

Arum triphyllum: all the seretions are acrid, the nostrils and lips are sore. There is thirst, but drinking causes pain. The nostrils are sore, and there is contant desire to bore the finger into the nose.

Lemna Minor: A catarrhal remedy. Nasal polyps, swollen turbinates. Atrophic rhinitis asthma from nasal obstruction, wrose in wet weather.

Kali lod. Constitutional remedy, starts with sneezing, pfofuse watry acrid coryza with pain in frontal sinus, cold travels downward to chest, laynhx feel raw, violent cough

Sulphur – found to be helpful in asthma alternating with recurrent skin eruption, fluent burning coryza, worse when outdoors, stops when indoors. Frequent sneezing.

Teucr ham marum: Catarrhal condition of both anterior and posterior nostrils. Mucous polyps. Chronic catarrh, crawling in nostrils with lachrymation and sneezing. Coryza with stoppage of nostrils.

Solidago: Hay fever. Spasmodic sneezing with runny nose. Copious, watery nasal discharge, worse from the odour of flowers, even thinking of flowers increases the discharge. Oversensitiveness to odours.

Sanguinaria nitrate: Acute clods with sneezing, profuse discharge, burning and rawness in posterior nares, husky voice (must clear throat before speaking).

Pothos: Asthma fromdust exposure (Kalic, Batta, O\, Solanum)

Asthma relieved by stool.

Psorinum: nose dry, coryza, with stoppage of nose, chronic catarrh; dropping from posterior nares, Asthma, with dysponea; worse, while sitting up; better, on lying down.

Fresher’s day was celebrated in Devs Homeopathic Medical College, Ankireddy Palli, RR District on 24th Dec 2011 Saturday. On this occasion Navigator Batch (seniors) extended warm welcome to the Titan Batch (Fresher’s) in a beautifully decorated college Auditorium.

Dr K.Siva Shankar former Additional Director, Govt of A.P, Dr.N.Vasudev, Vice-President Devs medical and educational society and Dr.M.Satyadev President, Devs Medical and Educational society. Graced the occasion with their benign presence. The dignitaries were given a floral welcome by the senior intermediate students Navigator Batch

The college mission of the year was declared by Dr.Siva Shankar.

Current health news centre was inaugurated by Dr.Vasudev.

Facing the ordeal of critical situation in medical emergencies

Introduction:

“No…no I don’tthink so….it is not possible because in emergency we need medicine with quickeffect. How is it possible with homoeopathic medicine?” this is opinion in thepeople around US.

Medical emergenciesconstitute an important part of the medical practice. Correct diagnosis andprompt and Appropriate treatment is essential to save life. Such emergenciesnot only test the knowledge but also the skill of the medical practitioner. Onehas to require not only presence of mind but also confidence to tackle themedical emergencies. In Medical emergencies every second counts. Providing urgent treatment to patients whosemedical histories and medications are unknown. Every experience of attending amedical emergency gives courage, experience and knowledge. It is not liketaking a long case for one hour and a short or acute case for 20 minutes.

Challenging situationfor homoeopath Physician in a medical emergencies are either to face theemergency or to escape it due lack ofconfidence, if he opts later hewill be losing reputation as a Physician, it will also be a black remark onHomoeopathic system, as already there is a misconception that homoeopathicsystem is only for chronic diseases not for emergencies. Knowledge,attitude and practice are very important for a physician in emergencies.Likewise he has to identify the problem within minutes and plan to give firstaid treatment till the patient is shifted to a well established hospital, apatient with severe abdominal pain and vomiting can be food poisoning, can be acase of acute gastritis, it can also be a case of ingestion of any pesticidewhich require gastric lavage and it is a case of medico legal which he has toreport to Police.

The following areimportant in emergencies .Situation at site of emergency, role of physician,cooperation of the Attendees. For a Private practitioner Situation can be atresidence of the patient or physician’s residence or at any public place, foran internee or for a physician who are attached institution has to face an emergency from alreadyadmitted patient or a newly admitted emergency case. In Medical Emergencies there is a fear and anxiety among patients near& dear. Crowd around the patient, making the patient more stressed,and obstructing the smooth working of the emergency services.

There is a lot ofdifference in types of Emergencies in rural and in urban areas , The types ofemergencies in rural areas vary like snake bite, Scorpion bite, accidents, stab injuries and suicides with pesticides etc along with common medical emergencies like acute abdomen like renal colic, acute gastritis pain, gastroenteritis , Cardio Vascular Accidents , Myocardial Infarction etc. In rural areas supportive staff,transportation, auxiliary medical help is lacking, in very critical conditions.In majority of rural places, referral hospitals are very far, transporting thecase to nearby hospital is difficult due to lack of transport facilities duringthis time we have to attend the case.

Conclusion: Object of this articleis to show that homoeopathic medicinesare able to cause quick effect when given in critical situations like various medicalemergencies, WeHomoeopaths have to take spot decision in selecting the remedy, as, time willnot wait for us, before arrival of ambulance what can a homoeopath can do? Hismotto is to Preserve life, by problemidentification and prevent thecondition from worsening, by selecting an indicated remedy at earliest.rightmedicine right time, we can save notonly the life of patient but prevent him from big disability. Our duty not onlygiving remedy also to do the general management tillarrival of ambulance or till hisreaches hospital. The vitalsteps in emergencies are Clearthe airway ,assess if the person is conscious / breathing ,Laying the person on his back on a hard surface , risingthe head level , to open his airway ,Check for breathing sound if notbreathing, start mouth-to-mouth breathing. Irrespective of systemwe practice learning how to perform cardio-pulmonary resuscitation (CPR) ismust to saves lives, Combination of rescue breathing& chest compressions ,done on unconscious patient , personssuffering cardiac arrest and in drowning/ asphyxiation/ trauma cases .²^.

My main objective towrite this article is to introduce the value of homoeopathic medicine inemergency cases. It is high time to start a P G course in emergency medicine inHomoeopathy. Even our students do not believe the efficacy of Homoeopathicmedicines in various medical or gynaecological emergencies until we show themthe cases treated by us.

If the homoeopathiccolleges are under the shelter of conventional system of medical college, ourfriends in other systems of medicine don’t have confidence in our system,instead of cooperation they discourage patient to take the homoeopathictreatment in emergencies, and even in their system of medicine scope is less infew emergencies

Case no 1: Suddenly, I heard door bell ringing continuously, I saw time it was 3 AM, with great hesitation by wiping myeyes , I opened the door, four peoplewith anxious looks were standing , a person was sitting on the floor, bhiya(brother ) please help, then I asked what happened ? Patient was a know facestays in our locality, he told me peshab nai hara (I am not able to pass urine,only with great difficulty I could pass few drops), he was restless, rolling onthe floor, please do something. Pressurewas building on me, I told them this is house I cannot do anything here, andone of the lady she shouted at me are you not a doctor? What for you studiedmedicine? Now time is 3 am how can we take him to hospital , we are all ladies,I told them I will call an ambulance , but for that also they were not preparedas 108 is meant for last stages and in govtinstitutions attention will be poor, to complete formalities it takes time. (in me also anxiety was building up ,my mind says at this movement that what can I do ? as he is elderly patientI asked him about his problem, he told he was having urine flow problem, doctor advised him to get operated. I presumed it could be BPH, I don’thave a catheter to insert nor even if it brought I don’t the experience ofinserting it as we don’t do regularly, best way is to give a medicine and sendthem to a local nursing home,) On examination bladder was full, I gave Aconite30c 5 DOSES and asked to take every half an hour.. Within 10 minutes of firstdose he passed feeble stream of urine, due over anxiety he took even seconddose after an hour he could pass normal stream of urine. He was relieved and I was relived.

Conclusion: in above caseof emergency situation was tense, at tenders were reluctant to take the patientto hospital, patients condition they were not able to asses. As physician’srole will be limited, this thing happen in majority of emergencies .In thiscase mental agony of a patient with urinary retention with full bladder can beunderstandable. Reasons for selection of Aconite are patient’s anxiety,restlessness.

Caseno 2:Patient aged about 51 years obese, male came with heaviness of chest, overanxiety, restless, was brought by his wife, on examination his pulse was 100pm, BP was unreadable, I advised him to go to cardiac centre as early aspossible, he has requested me to give some medicine till he reached hospital, Igave Aconite 30c 5 DOSES and asked to take every half an hour. To fetchambulance 108, it took nearly 30-40 minutes, he told me he was feeling better,but he went to hospital, he was admitted in ICCU. Case was diagnosed as MI,

Conclusion: In this caseas my teacher told me that first think of a worst condition and plan thetreatment in this case also mental agony of a patient with restlessness, substernal heaviness, was the reasons for selection of Aconite is patient’sanxiety, restlessness.

Case3 : A boy aged about 4years of age was admitted withhigh fever , for which he was given conventional type of treatment, feverreduced , after 6 hours he has become drowsy ad finally he became unconsciousand admitted in ICU, GCS score was 3, he belongs to a doctors family , who wasknown to me, doctor spoke to me and said only Manintal drip is been given,chances of survival are bilk , he request me to send the medicine, on the basesof congested face, Pupils dilated, Belladonna 30C, water dose I have sent indrop bottle and advised them to give every half an hour two drops of medicinein mouth by pulling lower lip. Herevered slowly, after 36 hours leg movements were noticed, he regainedconsciousness after 5 days. After that case was followed by Cal Caria carb 30C

Conclusion: In this casefever followed by coma, on the bases of congested face, Pupils dilated,Belladonna 30C, water dose was given.

Case 4: Patient aged 14 years, male old OPD Bronchial Asthma case,was better for 6months after Homoeopathic treatment, suddenly came to OPD withsever shortness of breath, wheeze, earlier he was on Kali Carb 200C. Onexamination of the patient Respiration rate was 30 PM, on auscultationbilateral wheeze was heard. He was given Carbo veg 30C, in water , one doseevery 15 minutes , two spoons, even after 30 minutes also condition was nobetter, Blatta Orientalis Q was add tothe prescription , 10 drops in 15 ml warm water to be given every 15 minutes. After 20 minutes respiration rateslowly came down, wheeze was reduced. Within an hour there was betterment inthe patient.

Before giving BlattaOrientalis Q we wanted to shift the patient to a hospital either fornebulisation or for oxygen. Giving both Carbo veg 30C and Blatta Orientalis Q we could avoid hospitalisation

Case5:Female aged about 27 years ,married, came withagonising pain in lower abdomen, difficult to pass urine, constant urge to passurine with pain, with few blood streaks in urine , patient was restless,Cantharis 30c , 5 doses were given askedher to take one dose every half an hourly and advised her to watch the pain, if she is better by 20% advised her to stay back in the house andreport back after 24 hours if not advised her to go to a hospital .She phoned me that after 4^th dose of Cantharis she passed multiple stones, paintotally reduced within 8 hours , which she collected they are shown below visual .

Conclusion:Inthis case most probable diagnosis was ureteric calculus Constant urgeto pass urine with pain, with few blood streaks, patient was restless. Cantharis30c, 5 doses were prescribed.

Caseno 6: A lady aged55 years had sudden fall after waking up and went into coma. She was brought tocity from Jangam. She was taken to local govt hospital and was admitted, case wasexamined by Neuro physician, they told the attenders that it is a case of CVA,utmost she may survive for 72 hours as the GCS was 3, there is no treatment forthis, As patients son was known to me I gave Arnica 30c in water dose asked torepeated every 4^th hourly. After 24 hours BP and pulse was slightlystabilised , Arnica 200c , was given , No response even after 48 hours, 72hours cut out time given by Neuro physician, was over , as right side isaffected I gave Causticum 200 C, wasgiven and waited for 48 hours , f or me I could not find much improvement, but

Attenders of patientwere positive. Absolutely there was zero cooperation from staff; staffs were inadequateemergencies were UN imaginable, only one PG on rotation was present. She diedon 8^th day.

Caseno 7: One at 10 pmmy own maid servant aged 75 years hadsudden complained of severe headache , followed by vomiting, became drowsy, BPwas fluctuating , pulse rate was high , an elderly lady , I thought it could bea CVA, I gave Arnica 30c in water dose and shifted her to a nursing home, where she went into coma , provisionaldiagnosis was CVA , she died around 3AM .

In above two cases Icould not do much, both cases were diagnosed as Cerebro vascular accidents i.e.cerebral Haemorrhage.

Case 8 :It 1 pm I was called by aunt urgently, when I reached the scene from college, my uncle aged about 70 years,was behaving oddly, moving from one end to other end, since half an hour. Icould not get any information except he was a diabetic, Hypertensive, I was nothaving BP apparatus to measure BP, and He was not allowing me to touch. Pulsewas 90PM.he was on oral medication for diabetics. Options were many for abovecase but time is running out, I phoned 108 ambulance, luckily glucometer was there I tested RBS was45. He was forcible given sugar water and sifted to hospital by 1.30 PM. Eventhough we are in medical profession we don’t carry emergency kits with us. Forhim to recover it took 36 hours.

Psoriasis is a common, chronic, recurrent², non-infectiousgeneticallydetermined, inflammatory skin disorder of unknown cause,papulosquamous skin disease that may appear at any age and affect any part ofthe skin.

25cases were studied(20male & 5female), the diagnosis was made clinically; some cases werereferred by Dermatologists. In 6 Cases BMI was above 30, 6 cases were US based,in 3 cases sever negative impact was present, the types of Psoriatic cases were as shown in table below. Improvement Scale :47.6% cases gotabove 70% improvement, 28.5% cases got above 50% improvement, 14.4% cases gotabove 30% improvement, 9.5% cases got above 10% improvement, The medicinesfound useful were Calcaria Carb,Graphites,Nat murur, Pulsatilla, Ars Alb, Thuja oc, Sepia, Acid Nitric, Sulphurand Thyroidin. Following nosodes weregiven Tuberculinum, Syphilinum and Carcinosin. The potencies used were in 30,200 and 1M. The 30 potency was repeated weekly, 200 potencies were repeatedfortnightly and 1M potency was repeated after a month. Nosodes were given in 1Mpotency. Along with treatment diet advice, Psychological counseling was given.

Conclusions:

It is a common belief that Psoriatic diseases difficultto cure, but if you take detailed life space investigation, read the mind ofpsoriatic patient we can overcome the challenges in treating Psoriatic diseases. Inconventional system of medicine treatment isunsatisfactory as the ‘cause’ is unknown; hence the treatment is onlypalliative; In Homoeopathic system psoriatic skin diseases canbe treated successfully. Guttate psoriasis, Pustular psoriasis^,Psoriatic Tongue and Erythrodermic psoriasis which are successfully treated by the author,details of cases are published in reference cited journals

Abstract:

Introduction:Gallstones are a major public health problem in alldeveloped countries. Although gallbladder disease had traditionally beenconsidered an adult condition, the prevalence has been rising in the pediatricpopulation. Baby aged 5year came with multiple Gall stones wastreated successful with Lycopodium and CarduusMarianus. Treatmentwas guided by ultra sound scan of abdomen before and after treatment. It is anEvidenced based Homoeopathic treatment.

Introduction:

Cholelithiasisis defined as “the presence or formationof stones in the gall bladder”. Cholelithiasis incidence ranging from 0.15% to 0.22%.^.Cholelithiasisis commonthroughout the adult population, affecting as many as 25 million Americans andresulting in 500,000-700,000 cholecystectomies per year.

The Cholelithiasis prevalence has been rising in thepediatric population. Gallbladder disease is much rarer in children, with 1.3pediatric cases occurring per every 1000 adult cases .A population-based studyestimated that the prevalence of gallstones and biliary sludge in children is at1.9% and 1.46%, respectively²_.Pediatric patients undergo4% of all cholecystectomies. Cholecystectomy is the procedure of choice forsymptomatic children with Cholelithiasis, regardless of age ^{2, 3}_. Cholelithiasis can occur at any age, includingprenatally, but is most common during puberty. Females are at significantlyhigher risk, with an overall 4:1 female-to-male predominance that increases to11 to 22.1 during adolescence.

Thecommon causes related to predisposing factors are Hemolytic disease, hepatobiliary disease,obesity, prolonged parenteral nutrition, abdominal surgery, trauma, sepsis, andpregnancy. In children one fourth of total Cholelithiasis cases are Calciumcarbonate stones, remaining 48% of gallstones in children belongs to Blackpigment stones. They are formed when bile becomes supersaturated with calciumbilirubinate, the calcium salt of unconjugated bilirubin. Black pigment stonesare commonly formed in hemolytic disorders and can also develop with parenteralnutrition. The complications of Cholelithiasis in children are similar to thosein adults.²

ACase: Baby aged 5year came with following complaints

Complaints:Pain in the abdomen on and off, pain in the throat with recurrent fever. withmultiple Gall stones.

Presentingcomplaints : Pain in the right hypochondria on and off since a year shifting from right toleft, fullness of abdomen, on and off pain in the throat < by eating Chocolates, she has great desire to eat.

Pasthistory: nothing particular

Treatmenthistory: took conventional medicine for above problem.

Familyhistory: mother is suffering from Migraine.

Personalhistory:

Appetite: Normal

Thirst: normal

Desires: Chocolates

Aversions: - Nothing Particular

Bowel movements: Normal.

Generalphysical examination & observations : normal built with20 KGs; child is fussy in nature, onexamination of throat : no congestion only chronic enlargement of tonsils.

8/9/2009:NuxVomica 200,3doses was given and 15 days Rubrum was given.

25/09/09:nochange in her complaints, 7 days Rubrum was given

09/10/09: no change inher complaints as per the Repertorisation, Lycopodium covered 7 out of sixsymptoms, I gave Lycopodium 30 C 3doses were given, 15 days Rubrumwas given.

24/10/09: pain in throatreduced, appetite no improvement, pain right hypochondrium was persisting, 15days Rubrum was given

19/11/09 appetite improved. Painright hypochondrium was persisting, fullness of abdomen was slightly better,Tuberculinum 200C, 3doses were given, 15 days Rubrum was given.

28/11 / 09 she camewith pain in right upper quadrant of abdomen, bowel movement was normal,regular. Cardus M 6C, 2pill twice a day for 1O days followed by Rubrum 20 Days,

25/12/09 : Weighthas increases to 21 kgs, General condition was normal, repeated Cardus M 6C , 2pill twice a day for 1O days followed by Rubrum 20 Daysadvised them to go for Ultra sound Scan, Liver function Test

17/01/2010: Generalcondition was normal Ultra sound Scanwas normal as shown below.

Liver function Test Within normal limits. Advice the father of thepatient to come after 2 months for review.

Reasons for selecting the remedies:

NuxVomica was selected as case has come from conventional system of treatment; ithas covered five out of seven symptoms. Lycopodium was selected as it has covered six out of seven symptoms. It isa right sided remedy ,Tuberculinum was prescribed as well indicated remedies are not givingdesired results, Carduus Marianus 6C was given as it is a very important remedyfor Gall Stones , there was lot of pressure on me as said patient was relative of local Gastroenterologist , there was fear ofobstruction, parents were afraid of going for Surgery. For this reasons I have changed the remedies frequently. But child got desired cure.

Conclusion: estimatedthe prevalence of gallstones and biliary sludge in children at 1.9% and 1.46%, respectively.²in children very common presentation will be pain in right upper quadrantof abdomen, most of the Physicians don’t dream of this Cholelithiasis condition.In majority of cases we can relive the pain by our homoeopathic remedies.Sometimes we may not get expected results, only ultra sound Scan can give usthe direction to proper treatment. The above case is one example, anorganopathic remedy helped to dissolve the multiple stones. In children we havedepend on only objective symptoms only as children are not in position to givetheir subjective symptoms.

A patient aged 50 years ,male, 73-3 KGs weight , occupationbusiness, diet was non veg, came to my clinic on 8th Jan 2011, with a stubbornfungal growth on middle figure (as diagnosed by dermatologist ) as shown invisual , area was very sensitive , pain on folding,

on 8th Jan 2011Bacillinum 1M One dose, Rubrum for15 days was given, without any benefit, AS he was a business man he was feeling guilty to sit in the counter, fingers were very sensitive to touch both the symptoms were present in both Lachesis and Cyclamen europaeum, Lachesis 0/1 for 11 days followed by 15 days Rubrum was given there was no relief and Lachesis 0/3 were given for 11 days followed by 15 days Rubrum was given without any benefit. Since 3 months therewas no improvement, I asked patient to get biopsy of the lesion, for which heagreed and the report is as show in the visual. The nosological diagnosis was not clear clinicallyand even in the biopsy report diagnosis options were Tuberculosis Verrucosa cutis,Lupus Vulganus, and Scrofuloderma.

As a last resort on 24/03/11. Carcinosin 1M was given, and Rubrum for 15days were given to my surprise lesion was less by fifty percent, Rubrum for 15days was given, he came after a month with a smooth figure as shown in visual .Thiswas turning point.

Dr Alois Alzhemer noticed changes in the brain tissue of a woman who had died of an unusual mental illness. Her symptoms included memory loss, language problems, and unpredictable behavior. After she died, he examined her brain and found many abnormal clumps (amyloid plaques) and tangled bundles of fibers (neurofibrillary tangles). Plaques and tangles in the brain are two of the main features of Alzheimer’s disease. The third is the loss of connections between nerve cells (neurons) in the brain.1

Every 71 seconds, someone develops Alzheimer disease, it is estimated that about five million Americans and 30 million people worldwide suffer from Alzheimer disease. It affects about 10 percent of people ages 65 and up, and the prevalence doubles roughly every 10 years after age 65. Half of the population ages 85 and up may have Alzheimer disease.

Alzheimer is a slowly progressive disease of the brain that is characterized by impairment of memory and eventually by disturbances in reasoning, planning, language, and perception. It the results from an increase production of beta-amyloid protein in the brain that leads to nerve cell death. It is a progressive and irreversible disorder.

REMEDY SELECTED : 02092005 Pulsatilla 1M one dose and placebo for 15days, no improvement , placebo for 15days, 16092005 no improvement , Pulsatilla 1M one dose and placebo for 15days was repeated, 18102005she came after a month with following improvement, constipation relived, appetite slightly improved and able to get sleep. placebo for 15days was repeated. 21122005 two months after she came and said fear and dullness was better, placebo for 15days was repeated. Patient did not turn up after.
FOLLOW UP
CASE SUMMARY: case belongs to lower middle class, widow, dependent, with multiple social and psychiatric problems, as she is a burden to her son; leucoderma has become a social problem for her and for her grand daughters who are to be married. In this case I could not cure her health problems; she was slightly recovered from depression with homoeopathic treatment. Geriatric problems are very difficult as it is inter linked with multiple social and psychological problems, as case is with multidimensional problems it takes long time to recover.

Geriatrics is the branch of medicine that deals with the diseases of old age. The study of physical and psychological changes that occur in old age is called “gerontology”. At present, there few such specialty in India though 20 per cent of doctor visits, 30 per cent of hospital days and 50 per cent of bed-ridden days relate to the elderly. In 1947, when India became independent, the average life span of an Indian was around 32 years. Now it has gone beyond sixty years. The elderly population in India which was 7.7 crore, as per Census 2001, is projected to be around 9.5 crore in 2011. it is projected to further go up to 17.3 crore in 2026. Rise in the share of elderly in total population poses multiple challenges. These challenges, like for the younger generation, who can utilize the vast repository of knowledge that our elderly possess. Simultaneously, our senior citizens too have to think of ways to creatively and usefully occupy themselves.3 , 1, 2

Social problems like Poverty, Loneliness, Dependency, Isolation, Elder abuse, Generation Gap are common. 2/3rd of Indian elderly live in rural areas. Geriatric problems are related to socio-economic status, health and living status One-third was reported to be living below the poverty line, Overall 75% of the economically dependent elderly are supported by their children and grandchildren. Chronic interfamily verbal abuse are the most common, followed by financial abuse, physical abuse, and neglect by adult children, daughters-in-law, spouses, and sons-in-law. Per capita costs for acute and long-term health care services are beyond reach.2,1
The following are few of the health problems that are observed in day to day practice - 88% visual impairment, 40% joint disorders like Osteoarthritis - Falling is a very common and often, a serious problem for older persons as it results in injuries such as broken bones and cuts, and the need for hospital care. 18.7% neurological complaints, 17.4% Cardio Vascular Diseases, 16.1% Respiratory disorders, 13.3% skin, 9% Gastro intestinal disorders, 8.5% psychiatric problems, 8.2% Hearing problems and 3.5 % bladder control problems are found in 1 out of 3 people over the age of 65.. 1.7 million deaths in this age group are due to chronic diseases, 36.3% were due to heart disease, 22.5% to cancer, and 8.2% to cerebro vascular diseases, Lung cancer has the second highest incidence in men at all older ages and in women aged 55 to 75.Colon cancer is the third most common cancer in men aged 55.1
Psychological problems commonly observed are dementia, anxiety, depression, fear, somatoform disorders, psychosis, sexual problems like sexual dysfunction, substance abuse and sleep problems like loss of deepest levels of non-REM sleep, more daytime napping among the elderly, sleep apnea. The common cause of these problems are retirement, loss of personal identity, disruption of daily routines, death of a spouse, family members, or friends, changes in social and financial circumstances. Alzheimer's disease plus vascular dementia are common. Alzheimer's disease is the most common type and it accounts for about 2/3 of cases of established dementia. The incidence of seizures increases throughout the course of dementia. Hallucinations, delusions, or paranoia occurs in about 25% of patients with dementia.

Management:
In the treatment of Geriatrics, Homoeopathic system has an edge over the conventional medicinal system. According to Dr Hahnemann, physician should clearly perceive what is to be cured in diseases; and to achieve this in Geriatrics, history and physical examination sometimes needs to be modified. Because a complete history may not be obtainable, elderly patients may present with many nonspecific symptoms making it difficult to undertake the case taking. Hearing or vision loss, common in the elderly, can also interfere with the case taking. Elderly patients may under-report symptoms (E.g.: dyspnea, hearing or vision loss, problems with memory, incontinence, gait disturbance, constipation, dizziness, falls), which they consider a part of normal aging. As a physician we may need to focus more on the physical examination to elicit objective symptoms. Elderly patients may have difficulty recalling all past illnesses, hospitalizations, operations, and drug use, have to obtain these data from family member, aide or medical records. Past medical history when reviewing the physician should ask the patient about diseases that used to be more common, past psychiatric care, including psychotherapy, institutionalization, and electroconvulsive therapy and the use of psychoactive drugs or antidepressants. Drug history & Nutrition history are also important.

Trichotillomania, which is classified as an impulse control disorder by DSM-IV, is the compulsive urge to pull out one's own hair leading to noticeable hair loss, distress, and social or functional impairment. It is often chronic and difficult to treat. The hair-pulling behaviour can occur during both relaxed and stressful times, but people with Trichotillomania often experience a mounting sense of tension before hair pulling occurs or when attempting to resist the behaviour. Hair pulling was first described in the literature in 1885, [2] and the term Trichotillomania was coined by the French dermatologist François Henri Hallopeau in 1889.1.The lifetime prevalence is estimated to be between 0.6% (overall) and may be as high as 1.5% (in males) to 3.4% (in females).
Trichotillomania is usually confined to one or two sites, [1] but can involve multiple sites. The scalp is the most common pulling site, followed by the eyebrows, eyelashes, face, arms, legs, and pubic hairs.2The classic presentation is the "Friar Tuck" form of vertex and crown alopecia . Most oftentimes, hairs are removed when the mind is occupied and the hands are idle like lying in bed, reading, watching television, etc.
CASE SUMMARY: Hair pulling 6year duration took conventional medicines without any benefit, Sepia gave good results, she took 6 -7 months treatment and stopped for six months the problem didn’t recur. She gained weight her physical and psychological well being was noticed.

A CASE OF TRIGGER FINGER, Dr G R Mohan

INTERDUCTION: Trigger Finger or snapping finger or Digital Tenovaginities Stenosans are SYNONYMS. In this condition when patient attempts to open the closed fist the affected finger remains in flexed position (locked) and it has to be passively stretched with other hand. It goes into extension with click. More commonly seen in females with a frequency 2-6 times that observed in men. (1). The most commonly affected digit is the thumb, followed by the ring, long, little, and index fingers Increased incidence in the dominant hand is observed. Involvement of several fingers is not unusual. It is a result of nodule formation on flexor tendon. It occurs as complications of either simple Tendon Synovities or Rheumatic Arthritis. Nodule is usually small. The Nodule commonly is trapped under the Meta carpo Phalyngeal ligament as the tendon flexes. The finger is them locked in flexon and active extension is impossible. To unlock the finger passively pressure is used. Sudden snapping (Trigger) of the digit occurs, finger is extended. Locking or catching during active flexion-extension activity may need passive manipulation to extend the digit in later stages .Stiff digit, especially in long-standing or neglected cases ,Pain over the distal palm ,Pain radiating along the digit are symptoms. Systemic causes of Trigger Finger are collagen-vascular diseases, including Rheumatic Arthritis, Diabetes, psoriatic arthritis, amyloidosis, hypothyroidism, sarcoidosis, and pigmented villonodular synovitis.
Treatment is not satisfactory in other systems of medicine. Only cartico steroid injection or operation in which incising the mouth of the fibrous flexor sheath longitudinally (1).

CASE:

Patient by name Ms. J. aged 45 years, married with two children without any history of Diabetes or Hypertension. She was diagnosed as Trigger finger by local orthopedic surgeon. She came to me with following symptoms to avoid Surgery. Pain in Rt. Little finger since one year – pain drawing type, aggravated by initial movement, aggravated early morning, inability to extended the finger, only passively she has to extend the finger with other hand and in general aggravated in damp weather. Sleep: N, appetite: N, Bowels: Regular, Urine: free, perspiration: Normal and menses: attained Menopause.

F/H Arthritis

O/E Slight swelling the finger with tenderness.

There are no much symptoms, only probable cause is over use of her fingers in daily dowe mixing. On 22/6/05 Rhus Tox 200 C, 3 doses. 4th hourly one dose was prescribed followed placebo for 5 days. Very slight improvement was noticed. 30/06/05 Rhus Tox 1M, 1 dose followed by placebo 7 days was prescribed. After this course pain reduced slightly only, inability to extend finger is as usual. Nodular condition, involvement of flexor tendons (3) Ruta Graveolen 200 C, 3 doses, 6th hourly one dose followed by placebo for 7 days prescribed. After Ruta.G patient was able to extend the finger little bit and pain reduced. Ruta G 1M, 1 dose was given along with placebo for 15 days. Patient reported back and she was able to extend her finger without any help, pain totally reduced. A complimentary drug Cal Phos 200 C, 3 doses was given lastly. The symptoms did not return even after one year.

DISCUSSION:

This is a case of few symptoms. Rhus Tox only helped her to some extent. Ruta G is having tendency to the formation of deposits in the periosteum, tendons, and about joints, especially wrist it acts upon the periosteum, cartilages and Flexor tendons, was tried up to 1m potency with good response even after one year there were no recurrence of symptoms. Case was diagnosed on clinically only. Most of the Pathological and radiological investigations will be normal.

Osteomyelitis is inflammation and destruction of bone caused by bacteria, Mycobacterium, or fungi (1)
Osteomyelitis is a particularly tragic preventable disease; it was once common, particularly among the poor, disease of the disadvantaged children of the developing world (3).
Haematogenous Osteomyelitis and Traumatic Osteomyelitis are two types. In Haematogenous Osteomyelitis bacteria reach bone through the circulation, Traumatic Osteomyelitis particularly follows road accidents and war injuries, in which bacteria reach bone through a badly treated open fracture or wound. And is also seen in inadequate wound toilet and through unsterile theatres. 80% spread is by open wounds and by hematogenous, Trauma, ischemia, and foreign bodies predispose to osteomyelitis. It is often polymicrobial.

The metaphysis of a long bone is the usual site. Osteomyelitis in the proximal tibia, the distal femur, the proximal femur, the proximal end of the humerus, the distal radius or ulna, the distal tibia, or the calcaneus. But any bone can be involved, and sometimes several of them at the same time. (3)
Osteomyelitis tends to occlude local blood vessels, which causes bone necrosis and local spread of infection. Infection may expand through the bone cortex and spread under the periosteum, with formation of subcutaneous abscesses that may drain spontaneously through the skin (3)
Common symptoms are localized bone pain and tenderness with constitutional symptoms like weight loss, fatigue, fever, and localized warmth, swelling, erythema, and tenderness.(in acute osteomyelitis). In Chronic osteomyelitis bone pain, tenderness, and draining sinuses and without constitutional symptoms, it takes months to many years (1)
Diagnosis
CBP, TWBC, ESR or C-reactive protein (will be elevated)
Culture of bone (Will be positive), abscess,
X-rays, MRI, or radio isotopic bone scanning’s-rays become abnormal after 2 to 4 wk, showing periosteal elevation, bone destruction.
Homoeopathic concept:
It can be One sided local-surgical or non surgical. Needs constitutional treatment to remove the fundamental cause. On the phase of disease it can be psoro –syphilitic as there will be bone necrosis with formation of subcutaneous abscesses.

A case of chronic Osteomyelitis

Mr. .M, 27, M, Religion: C came to our OPD on 030303 with following complaints.
Pain in the left upper am, pricking type, pain is aggravated at night, patient in general oversensitive,
On examination we found multiple sinuses, offensive discharging of pus & blood, discharge of bone pieces at times. Local tenderness was present.

Physical generals:
Appetite- 2 meals/day.
Thirst- Normal, Bowels: regular. Sleep: refreshing, Dreams: Nothing specific.
Perspiration: Nothing specific
Thermal reaction: Sensitive to cold air
On examination we found multiple sinuses, discharging pus &blood, discharge of bone pieces at times,
C/o Physical generals: Sensitive to cold air

PAST HISTORY
History of similar complaint, started first in the tibia, underwent surgery and it healed slowly in 2 years.
FAMILY HISTORY: Nothing particular
PERSONAL HISTORY:
Middle school drop out, Occupation- no,
Marital status- unmarried.

Investigations:

Blood report: dated 030303
Hb: 10.9grm/dl
Total RBC: 3.78 mill/cumm
Total WBC: 18000 mill/cumm
N89, L16, E02, M02.ESR:1st hour125, 2nd hour 140.

X-rays as shown below:

Class of disease: One sided local-non surgical
Surgery not helped him; he was in need of constitutional treatment to remove the fundamental cause. Hence it comes under non surgical. The presentation of disease is only on the local part with few symptoms, hence one sided –local. Remedy selected was Hepar Sulph Reasons for selecting Hepar Sulph were offensive discharge, over sensitiveness, sensitive to cold air, it is a good remedy for supportive stage with already formed pus, to remove the sequestrum and it helps in healing.

First Prescription on 030303: Hepar Sulph 30c, 3doses, Rubrum (40) 4pills 12th hourly, for 15 days, advice to Clean the area with calendula external daily.
Follow up:
On 210303: no much change only offensiveness of discharge was less. Total count of WBC has come down, ESR was little lees. Hepar Sulph 30c, 3doses was repeated along with Rubrum (40) 4pills 12th hourly, for 30 days. (table1)
On 100403 patient reported and said pains is less by 25%, offensiveness of discharge and discharge was less but Total count of WBC has shot up to 11800 and ESR was little lees comparative to previous readings. Rubrum (40) 4pills 12th hourly, for 15 days and Calcaria Sulph 6x biochemical tablets were given 4+4 12th hourly.
280403: no much change, Hepar Sulph 200c, 3doses, Rubrum (40) 4pills 12th hourly, for 30 days.
On 230703 came with blood report Total count of WBC came down to 8900 and ESR was little lees. Pain and discharge was less. Still discharge was present a dose of Syphilinum 1M,1dose was given along with Rubrum (40) 4pills 12th hourly, for 15 days and after 15days Calcaria Sulph 6x biochemical tablets were given 4+4 12th hourly.
Patient reported on 140903 with following presentation pain less, discharge is not seen, slight tenderness at the sight, we advised him to go for X ray and routine blood examination. Hepar Sulph 1M, 1doses, Rubrum (40) 4pills 12th hourly, for 30 days were given.

221003 patients came with blood report as shown in table (1), Total count of WBC again shoot up to 10200 but ESR was very lees. No pain and discharge was very less. Slight tenderness was seen on deep pressure. Rubrum (40) 4pills 12th hourly, for 30 days were given.

On 221203 he came with general wellbeing. Hepar Sulph 1M, 1doses, Rubrum (40) 4pills 12th hourly, for 30 days were given. We advised him to come with X - ray.
As patient was better, he was not maintaing the regularity in taking medicines
240404 patients came with a blood report and X-ray as shown below, total count of WBC has shot up to 10200 and ESR was 32mm only, in X-ray there was good improvement. Hepar Sulph 1M, 1doses, Rubrum (40) 4pills 12th hourly, for 30 days were given.

Summary: There is a misconception that homoeopathy may not work for deep seated pathological conditions. The above illustrated case is an example how homoeopathy does wonders.

HEPAR SULPHURIS:
It is suited to the psoric, scrofulous, diathesis. Debilitated subjects.
Tendency to suppurations. Slow torpid constitutions with lax tissue and light hair, great sensitiveness to slightest contact of ulcers, eruptions and parts affected. Great sensitiveness to all impressions. Cold, dry winds. Injuries. Abuse of Mercury. Suppressed eruptions.Unhealthy skin, every little injury suppurates. Abscesses, suppurating glands are very sensitive. wants to be wrapped up warmly. Sticking or pricking in affected parts. . Great sensitiveness to slightest touch. (2)

It used to wake me up by 5AM with out an alarum clock; it was with us for nearly a decade, died due to non diagnosed central nervous disease. It had a painful death. Kaniyadanam – The Kaniyadanam involves the bride sitting on the father’s lap, and being given away to the groom. The priests from both sides of the family give an elaborate introduction of the bridge and the groom’s lineage & ancestors before proceeding with the Kaniyadanam ritual.
Thirumangalyam – The climax of the entire wedding ceremony, the groom ties the knot while the bride is seated on her father’s lap. This is followed by Saptapadhi where the bride and the groom walk around the holy fire.
The wedding was again attended by all our friends and relatives who were all present until the very end of the ceremony. I owe thanks to all for taking time out of their schedules to attend the ceremony to bless the couple.

Wedding – Red & Gold Theme – The wedding rites started off at 4.30 AM on Mar 6th, 2011 with the Ankurarpana and Varapuja ceremonies followed by the Kasi yaatrai, Maalai Maatral and Unjal ceremonies. Wedding was done in traditional Iyer wedding ceremonies.
Maalai Maatral – This interesting ceremony involved Ashmita and Rakshith exchanging red orchid garlands thrice. It was fun watching the garland exchange as it resembled something akin to a basketball match where the groom was dodging the garland.
Oonjal – The garland exchange was followed by Oonjal or the Swing ceremony, where Rakshith & Ashmita were seated in a swing and given a mix of milk & banana by the ladies on both the groom’s & the bride’s side. The ladies sing to ward off “evil eye” as the bride and groom are seated on the oonjal.

Reception – Blue Theme – The reception decoration & garlands were all centered on the theme set by my daughter Ashmita. She defined the theme as BLUE, blue being her & Rakshith’s favourite colour. Apurva, my niece and my family then successfully executed the concept, ensuring that even the garlands and the bridal bouquet had blue flowers in them. We ensured that the theme reflected everywhere in the venue, the stage, the chairs, Rakshith & Ashmita’s attires and well the cake & the flowers. The reception was attended by everyone close to the families, right from my childhood friends, closest relatives to my professors & colleagues with over 800 people. The reception was a grand success, and I owe the success to Mr. Premkumar of Shanmuga Caterers, Mr. Sivaram of Annapurva Events and Mr. Mohanlal (Orchestra).
A universal color, Blue has a calming and soothing effect on the psyche. Blue is associated with intelligence, stability, unity and conservatism.

Ashmits Pellikuturu Function

Pellikuturu Function – The Pellikuturu function started around 10.30 AM on 5th Mar, 2011 at Dhola-ri-Dhani right after the thread ceremony for the groom. My aunts, cousins and nieces & their friends were all actively involved in applying haldi & besan paste to the bride and blessing her. We were lucky to get Dhola-ri-Dhani, which truly helped us transform our vision of a perfect wedding to a reality.
The Pelliuturu function or the Haldi ceremony as it is more popularly known as, is a ceremony where the bride and the groom go through a elaborate ritual of ceremonial bath. The aim is to purify and prepare them for the wedding rites. In the olden days, this ritual was more elaborate with the bride being bathed in milk or sandal – rose scented waters but now due to time constraints these are sadly going extinct.
This was followed by Aarti where the female relatives all gave aarti to Ashmita and Rakshith to bless them.

Mehendi & Sangeet – An intricate part of the weddings these days, my daughter Ashmita’s Mehendi and Sangeet ceremony was held on 3rd March 2011 at our residence, OUR NEST. Attended by a small gathering of relatives & friends (~150), the mehendi was done amidst classical Rajasthani songs & dance. My nieces and cousins visiting from the US were enthusiastic participants in the Sangeet ceremony.
The ceremony started late evening and it took around 4 hours to complete the bridal mehendi alone. Mehendi signifies the strength of love in a marriage...and there is a belief that the darker the mehendi shade, the more stronger is the love between the couple. And since music is the soul of every Indian wedding, the Sangeet ceremony is more of an entertainment for relatives & friends.

Psychological distress in Prolapse inter vertebral disc.

The overall lifetime prevalence of back pain exceeds 70% in all industrial countries and the consequences include loss of 1.4 working days per person / per year; 10-15% of all sickness absence; Approximately 25% of the populations between the ages 40 to 50 have shown evidence of a Prolapse Inter Vertebral Disc on MRI scan, most of which were a symptomatic at the time. Study psychological distress in PIVD cases. Methods: A Clinical study of 81 cases of Prolapse Inter Vertebral Disc was done at APHA Super specialty clinic, Hyderabad and private clinic from 2006-2009 Results: 17.28% belongs to 45-49 years age , followed by 14.81% in 40-45 years age group, Gender females were more, Occupation: Non manual workers: 22, home makers: 27 manual workers: 32, social Group was: Low socioeconomic: 26,middle class: 37,upper middle class: 18, smokers were more than 50%, 90% were having psychological distress with apprehension about future, it was more in dropped out cases of 6.2% , remedies Lycopodium 30 , Kali carb 21 were leading, Result were as follows Relief of 76--99% in 30 cases, 51--75% in 30, 26-- 50% in 30, 0--25% in 10 cases and No dropped out were 6.2%. Ultimate answer was Pain reduction 90%, Work return 90%, increased physical function 60% and psychological wellbeing in 60 % cases. Discussion: Conventional systems of medicine have its limitation in tackling these disorders, and their approach in the treatment of disc prolapse is to provide temporary relief than a permanent cure through drugs and Microdisectomy, which a common Indian cannot afford. Homoeopathic system of medicine provides hope for a more permanent cure and relief if the treatment is planned methodically; in this system of medicine mental symptoms are given top priority. Conclusions: It found the holistic approach of Homoeopathy is very much pragmatic in dealing with PIVD disorders with psychological distress; still there is a lot to do to further validate the homoeopathic literature13
